Our goal is to help create and maintain a safe and healthy environment that supports the highest possible level of independence for the senior adult and to enrich the lives of those experiencing life changes.
Care Navigation services are currently provided at no cost to Pitkin County seniors over age 60, and their families.

Services
The Senior Care Navigation Program helps senior adults their families connect with options to insure safety and independence.
Successful Care Navigation is accomplished by:
- Identifying the needs, concerns and goals of the senior adult
- Assessing the personal health and physical environment of the senior
- Developing a care plan by determining needed services and making recommendations based on quality and cost effectiveness
- Locating and and coordinating the delivery of services and identifying potential resources
- Monitoring the provision of services and adjust services as required

Home Health
- Covered by Medicare
- Usually time limited up to 60 days
- You must be experiencing difficulty in leaving your home
- You must have a medical or skilled need
- Can be provided by an agency or an individual
- Do your homework on your provider, including reference and background checks
- Includes skilled nursing care, home health aides and homemaker services. Client can determine what aspects of service are needed from several times per week to round-the-clock care, home maker services, etc
- Usually private pay. A limited level of service can be provided through Medicaid for those who qualify. Otherwise, cost will vary depending whether services are provided by an agency or individual, and whether the individual is licensed or unlicensed
- Can be provided wherever the client lives – own home, assisted living or skilled nursing facility
- Can only be “enrolled” after all life-extending treatments have stopped (i.e. no more chemotherapy)
- Can provide nursing care as needed and some respite provided by volunteers but is not around-the-clock at home care
- End of life comfort care

Tax Relief
Locally in Pitkin County, there is a tax relief program for low- and moderate-income older adults who are residents within the county. Qualifications are dependent on annual income and the program is focused on those older adults with low and moderate incomes.
Locally in Pitkin County, there is a tax relief program for low- and moderate-income older adults who are residents within the county. Qualifications are dependent on annual income and the program is focused on those older adults with low and moderate incomes.
- Financial assistance is available for low-income senior residents in the form of partial property tax refunds from the City of Aspen, City of Aspen Utilities, Aspen School District and Pitkin County. Programs are administered by Senior Services. Call 920-5432 for information and a hard copy of the form.
